The template letter generated via this platform is intended as a guideline only to assist Healthcare Professionals (HCPs) in drafting motivation letters for submission to medical funders. The content is based on generalised clinical information available on the EduDoc platform and does not constitute a complete or final medical opinion.
HCPs are solely responsible for reviewing the letter, ensuring its clinical accuracy, and including all relevant patient-specific details in line with their professional judgement and ethical obligations. This may include, but is not limited to, the patient’s medical history, current condition, and treatment rationale.
Before finalising and submitting the letter, HCPs must:
- Ensure that all patient information is accurate, appropriate, and necessary for the purpose of the motivation;
- Remove any default text that is not applicable to the individual case; and
- Confirm that they have obtained the patient’s informed consent for the disclosure of their personal and health information to third parties, including medical schemes or funders.
This platform does not store or process patient-identifiable data unless specifically input and retained by the HCP in accordance with the Protection of Personal Information Act, 4 of 2013 (POPIA).
